FormsFor all planning and estate administration services, we ask that you complete some information to make our initial meeting more productive. This information is essential for us to provide you with the highly specialized legal advice you deserve.

Estate Planning Services

If you are interested in a comprehensive estate plan or have a loved one with special needs, we will need a bit more information from you to ensure that we are providing you with appropriate advice regarding all aspects of your plan. As part of our representation, we will review with you the registration of your assets and the beneficiary designations of your retirement accounts and life insurance so that they coincide with your ultimate planning goals. We also will review any tax issues that you may face now or upon your death.
 
Our Estate Planning Questionnaire requests information about you, your family and who you wish to nominate as your executor, trustee and guardian of any minor children. The questionnaire also request information about your assets, such as bank accounts, real estate and retirement accounts, as well as any issues or concerns you wish to address. We understand that the planning information we are requesting is extremely personal, so please be assured that it will always remain private and confidential in our office.
 
If you do not wish for us to prepare a personalized estate plan, we can create a basic plan for you. However, we will require that you sign a waiver and release of liability if the simplified plan does not adequately protect your assets and your needs. If this is the case, please choose the modified version of our Estate Planning Questionnaire–Short Form which only requests some basic information about you and your family and who you wish to nominate as executor, trustee and guardian of any minor children. It also requests that you provide us with an estimate of your overall net worth without any specific details about your assets.
 
After a brief telephone consultation, we will discuss with you which questionnaire to complete and provide you with a password to download the form. We would ask that you fax, email or mail it back to our office at least seven days before our initial meeting. Please do not worry if you are having trouble answering some questions in the questionnaire. We are here to help you and can discuss any questions or concerns during our first meeting.

Elder Law Planning Services

If you are interested in obtaining advice regarding how best to protect your assets from long-term care costs, we ask that you complete our Elder Law Questionnaire. We will need information about your assets and income to provide you with a comprehensive plan containing your planning options and Medicaid eligibility. We understand that the information we are requesting is extremely personal, so please be assured that it will always remain private and confidential in our office. We ask that you complete as much of the questionnaire as possible, but please do not worry if you are having trouble answering certain questions. We are here to help you and can discuss any questions or concerns during our first meeting.

Estate Administration Services

If you are named as the executor of a Will or need to act as the administrator of a loved one’s estate, your first responsibility will be to gather information and documents about that person, such as his or her assets, debts/liabilities and tax returns. We know that this process is extremely difficult and can be cumbersome if you do not have the proper direction. Therefore, we have created some documents to help guide you through the process.
